Food Insecurity Screening Survey

The Vermont Food Access and Health Care Consortium is running a second iteration of our food insecurity screening systems survey for Vermont health care practices. For this iteration we are looking for responses from hospitals, federally-qualified health centers, and primary care practices.

This year’s survey is designed based on responses from last year’s general survey (results here) and follow up interviews with health care practices about their food insecurity screening implementation. Answers to these surveys help us design resources, such as the Hunger Vital Sign explainer series, and understand the landscape of screening systems in Vermont as new federal policies begin to highlight HRSN screening in health care.

If you are at a hospital, federally-qualified health center, or primary care practice, we hope you can take a moment to respond to the survey - or share with the appropriate person to respond for your organization. The survey will be open through October 15th. We do cross check responding organizations at the end, so don’t worry about multiple responses from the same place.
