Outreach Grant

Bi-State Primary Care Association has been awarded a four-year Rural Health Services Outreach Program grant by the Health Resources & Services Administration (HRSA). This grant is part of the newly launched Healthy Rural Hometown Initiative (HRHI) track.

The Outreach Grant will support three pilot / demonstration projects around Produce Prescriptions, Medically Tailored Meals, and a Social Grocery at three Federally-Qualified Health Centers. It will also support the work of Bi-State Primary Care Association in sharing results from these projects and helping other providers develop food in health care programming. Clinical outcomes will be focused on reducing prevalence of heart disease, one of several HRHI priorities that focus on illnesses that disproportionately impact rural areas.
