The Food Access and Health Care Consortium is trying to better understand the landscape of food insecurity screening at Vermont health care practices. You can help (if you’re at a health care practice in Vermont) by completing this short survey!
Future iterations of this project may include health professionals not affiliated with a health care practice, community partners, and other details of tracking food access such as SDOH diagnosis coding / Z-codes . . . but in the interest of having a brief survey we kept this one focused.
